Enrolments
Enrolment Process
1. Complete an enrolment form online or come into clinic to fill out a paper form
2. We will request your notes from your previous GP - this can take up to 10 working days
3. We will call you to confirm enrolment and book your New Patient Nurse Appointment
4. Complusory New Patient Nurse Appointment - Initial health check up, and finishing setting up your file
5. You can then book with a GP/NP as needed

Name of previous GP practice for notes
An eligible individual is:
A New Zealand Citizen OR
Holds a resident visa or permanent resident visa OR
Is an Australian citizen or Australian permanent resident AND able to show that they have been in NZ or intends to stay in NZ for at least 2 consecutive years OR
Has a work visa and is able to show that they are able to be in NZ for at least 2 years OR
Is an interim visa holder who was eligible for publicly funded health services immediately before his/her interim visa started OR
Is a refugee or protected person
